How do I help patients who won't follow their treatment?

You have learned to manage chronic illness, to hold conditions steady over time. Where you feel defeated is the patient who will not follow the plan, or whose life is too tangled for the plan to fit. That frustration is honest. You know what would help, and you cannot make them take it. Here is the shift that helps: you are not failing when a patient does not comply. Often what looks like refusal is a life you cannot see, no money for the medicine, no ride to the pharmacy, fear, exhaustion, a hundred things. Your job is not to force the plan. It is to understand the real barrier and meet the person where they actually stand. With your next hard patient, ask one open question about their life instead of repeating the instructions. What gets in the way of taking this? Then listen. You may not fix it, and that is not yours to carry alone. But you will treat the real person, not the chart, and that is where change, when it comes, begins.
