Being behind on charting with that fear humming underneath, that you left something important out, is a heavy way to work. The dread is worse than the task. It follows you home and sits in your chest at night. Let us take some of the weight off. The fear is trying to protect you and your patients, which means it comes from care. But fear alone does not close the gap; a system does. You cannot hold every detail in your head, so stop asking yourself to. Put it on paper as you go instead. Start with one anchor: a short mental checklist of the few things that always must be recorded, and hit those first before anything else. Get the essentials down early and the terror of the missing detail loosens. You are a good clinician. You just need the record to carry what your memory should not have to.
