Why won't my team tell me about problems?
You keep finding out about problems too late, after they have grown. The reason is quieter and harder than bad luck: your people do not feel safe bringing you bad news, so they hold it until they cannot anymore. The silence is information, and it is pointing back at how it feels to tell you things. No one hides problems from a leader who makes it safe to be the messenger. They hide them from blame, from the sharp reaction, from the sense that bad news makes them look bad. Somewhere along the way the team learned that surfacing trouble costs them something. That is not their failing to fix. It is the room's, and you own the room. Change what happens when someone brings you a problem. The next time bad news arrives, thank the person before you do anything else, genuinely, out loud, and deal with the issue without turning on the messenger. Do that consistently and word travels. People bring problems early to leaders who make early news welcome. Your job is to become the kind of person it is safe to warn.
