My body is breaking down and my career hit a ceiling. Now what?
You gave your body to this work, and now it is asking to be repaid. You gave your years to this career, and it turned out smaller than you expected. Both facts are true. Neither one is the actual problem. The actual problem is quieter: you have been counting your worth in output. Patients helped, hours worked, cases closed. As long as the number could grow, you did not have to ask what the work meant. Now your body has put a limit on the number, and you are left facing the question you avoided. This is not a tragedy. It is an opening. You did not become less useful because you can do less. You are only now finding out whether the work ever meant anything beyond how much of it you could do.
Your body is not betraying you. It is sending the bill for years of treating yourself as a tool. The ceiling is real, but it is not your problem. Your problem is that you have measured your worth by how much you can still produce. Stop. Choose a different measure.

- A short list of what you will keep doing and what you will let go
- Language you can use with others when you scale back, that does not sound like an apology
