Why do approvals and duplicates slip past me?

You see the invoice. You do not see the hole in it. That is not because you are careless — it is because no one on your team has written down, in plain terms, what "checked" actually means. Volume does not forgive a missing method. Stop telling yourself you will pay closer attention next time. Attention is not a system. It wears thin by the fortieth invoice of the day. What does not wear thin is a fixed sequence, written down, that someone besides you also follows. This is not your flaw alone to carry. It is your pod's problem to solve. Build the checklist together. Split the checks between two sets of eyes. Then the duplicate gets caught before it leaves the building — not discovered after.

The fix is mostly a system to build and a habit to practice with others, not new facts to learn.

What’s really going on

You miss them because you are reading invoices, not checking them — two different acts. Attention runs out by the fortieth one. A written checklist does not. Build one with your team, split the checks between two people, and stop relying on your own focus to catch what a system should catch.

A moveWrite down the exact checks every invoice must pass — approval on file, no duplicate, amount matches — before anyone touches it.
A moveSplit the work: one person approves, a different person verifies. No one checks their own work alone.
A moveWhen an invoice skips a step, say so out loud to the team, not just to yourself.
A moveReview every miss as a group each week — find the step where it slipped, not who slipped.
A moveChoose the one checkpoint where mistakes keep happening, and agree as a team to slow down there.

What changes unlock by starting

  • Fewer duplicates get paid because two people check instead of one
  • The team shares one written sequence instead of relying on memory
  • Misses get caught before payment, not after
  • You stop carrying blame alone for what was really a missing process
