Why do I do a doctor's job without a doctor's title?

You took the lead role. Now you handle schedules and quality numbers. You still do a doctor's work — the judgment calls, the hard decisions. But you don't have a doctor's title or pay. That is your day, plainly stated. You did not fall into this by accident. You agreed to it — by staying quiet, or by taking pride in handling what others would not. That is not blame. It is the starting point. What you allowed, you can also change. Ask yourself this: is it fair? That question wastes your time. Fairness is not yours to control. What you ask for, what you refuse, and what you do next — those are yours. Start there.

What’s really going on

Your title will not catch up to your work by itself. Choose now: ask for a title and pay that match your duties, or stop carrying duties that are not yours. Waiting for someone to notice is not a plan. You already know this. Act on it.

A moveList every duty you perform that belongs to a doctor's job description. Write it down plainly, this week.
A moveTake that list to whoever sets titles and pay. Ask directly: will you match my title to my duties, or reduce my duties to my title?
A moveSet a date. If nothing changes in 60 days, decide whether you stay or look elsewhere.
A moveStop volunteering for extra tasks 'just this once.' Each time you do, you confirm the current arrangement works fine as it is.
A moveFind one outside person — a mentor, a union rep, or an advisor — who can tell you plainly if this is normal or being misused.

What changes unlock by starting

  • A clear written list of the gap between your duties and your title.
  • A direct, on-record conversation with your supervisor about title and pay.
  • A real decision point — stay, renegotiate, or leave — instead of indefinite waiting.
  • Less resentment, because you acted instead of just absorbing it.
One object, two jobs: a public answer to a real problem, and — the moment you start the chat — Aurelius’s live plan for your version of it.
