Why do tricky joints and dirty metal still ruin my welds?

You say you prep well. I believe you. That is not the question. The question is whether your prep fits the joint in front of you, and the metal in your hand. A hard joint is not solved by trying harder. It is solved by knowing its shape and changing your method to match it. Dirty metal is not solved by hoping your usual clean-up is enough. It is solved by knowing what the dirt is, and removing all of it before you strike an arc. You keep tripping on the same two things because you have never made either one its own lesson. You call it bad luck. It is not luck. It is ground you have not studied yet.
