Why does accuracy keep tripping up our team, not the data itself?

You and your team can gather almost anything now. That part works. Where it breaks is the last step — the one where someone actually checks that what was captured matches what was required. That step keeps getting skipped, by all of you, in the same place, under the same pressure. This is not a skill gap. It is a sequence problem. You move to the next task before finishing the one in front of you. Each person assumes the last person checked. No one did. The requirement was never the hard part — reading it fully, and holding each other to it, is. This will not be solved by one person trying harder. It is solved by the team agreeing, out loud, on where the check happens and who owns it. Decide that together now, not after the next error.

The team already understands the task; what's missing is a shared habit of scrutiny, so the page leans toward coaching action over teaching concepts.

What’s really going on

You are not failing to collect data. You are failing to finish looking at it before you act on it. The fix is not more tools — it is a pause your team agrees to take together, every time, before anything is called done. Choose the pause. Build it in. Stop trusting speed.

A moveBefore anyone submits data, have one person read it aloud while another checks it against the source, every time, no exceptions.
A moveState the accuracy requirement in your own words before you start work, as a team, not after someone questions it.
A moveBuild a two-minute pause into the process where each person reviews their own entry before passing it along.
A moveWhen an error appears, trace the moment haste took over — not who made the mistake.
A moveRotate one person each week to sample-check the team's work against the standard and report findings openly.

  • Blame drops; the process gets fixed instead.
  • Your word and your work start matching, consistently.
