When teams avoid accountability in post-incident reviews

Our engineering team has this pattern where we say we're going to do post-mortems after incidents but then we just fix the technical issue and move on because everyone's uncomfortable with the idea that someone might have screwed up. We've had three major outages this quarter and I can see us getting worse at the human side of these problems even as we get better at the technical recovery.
