When I check every med three times and still fear one mistake

I follow every rule, check the right patient, the right drug, the right dose. But I am haunted by the mistake I cannot see coming, the one that slips through anyway. So I check the label a third time and a fourth, holding up the line, because the thought of harming someone with my own hands scares me more than looking slow.
