When you ask questions but miss the deeper problem

I ask my discovery questions and get answers, but I stay on the surface too often. Sometimes I dig and find the real problem underneath; other times I take the first answer and move on, and miss it. I can feel there's a deeper question I should've asked, usually after the call is over.
