When you can write but can't structure a whole paper

I can write. Give me a page and I'll fill it with clear sentences. But when I try to shape a whole paper into something a journal would take — the argument, the sections, the way it all has to hold together — I fall apart. The writing is fine. The structure of a real paper defeats me.
