When you can't reach the patients who won't absorb it

I can teach most patients how to manage their condition and watch it click. But some people just glaze over, or nod along and clearly don't get it, or are too overwhelmed to take in a single word. I explain it three different ways and still lose them, and I don't know how to reach across that gap.
