When you do the doctor's work without the doctor's standing

I diagnose, I treat, I make the calls that keep people well, and I do it at a high level every single day. But there's a title I don't have and a standing that comes with it, and patients feel that gap even when they can't name it. I live in that tension, doing a doctor's work without ever quite being granted a doctor's place.
