When you manage the team by reacting to whoever's loudest

Someone quits, I scramble to backfill. Someone's unhappy, I put out that fire. I've never once sat down and asked what this team should actually look like in a year. I'm just reacting to whoever needs me loudest that day, and the team drifts wherever the wind blows.
