There's an opening and I know who'd actually be best for it, but they'd be a harder conversation and a bigger shake-up. So I hand it to the person who's easy and familiar instead. I keep choosing the comfortable pick over the right one, just to dodge the awkwardness, and the team quietly pays for it.
