When you run discovery but the real constraints surface too late

I can run a technical discovery call now and pull out most of what we need to know. But there's always some buried constraint that doesn't show up until we're halfway built, and then it blows the plan apart. I keep thinking I asked everything, and the killer detail still hides until it's too late.
