Why do I lose the thread when the caseload spikes?

You manage well enough on an ordinary day. That is not in question. The question is what happens when the cases double and your calm habits have nothing to hold onto. You call this losing the thread. Name it plainly: you had no rule for what comes first when everything arrives at once. This is not a flaw in your character. It is a gap in your system — and your team shares that gap with you. If each of you improvises differently under pressure, the whole pod loses the thread together. No one admits it alone, but it happens together. You cannot control when the spike comes. You can choose, today, what the three checks will be when it hits. Write them down. Agree on them together. That is the only thread worth holding.

What’s really going on

The thread was never in your head — it lived in a routine. When the load spikes, the routine breaks first, and you mistake that for your own failure. Fix this now, while things are calm. Agree with your team on the three things you always check first, no exceptions.

A moveBefore the next spike, write down the three things every case gets checked for — no more, no fewer.
A movePick the quietest point in your week and rehearse the triage drill then, as a pod, not alone.
A moveWhen volume rises, say out loud which cases you are deliberately not touching yet — tell your team, not just yourself.
A moveName one teammate to hold the master list when you're underwater, and let them hold it.
A moveAfter each spike, give your pod ten minutes to name what the rule missed, then fix the rule, not yourself.

What changes unlock by starting

  • A written triage rule your whole pod uses the same way.
  • Fewer cases forgotten or mishandled when volume rises.
  • Faster recovery after a spike, because the drill is already rehearsed.
  • Less private shame about 'losing the thread' — it becomes a shared, fixable system.
